In order to expedite our sign in process we are asking all adults attending this event with minors under the age of 18 to print and fill out the following forms for the Release of minors under 18.
Our regular adult release/waiver will be available at check in on the day of the ride. Minor forms will also be available that day, however we would greatly appreciate if you print and fill on out before your arrival to the park. Please contact us at 740-541-5226 with any questions.

 

Release for Minors Under 18 years of Age

One form per minor needs to be filled out.If you are the parent or legal guardian please read and fill out this form in full.This form does not need to be notarized if parents or guardian is present at sign in. However, if you are bringing a minor child that you are not the parent or legal guardian of, the parents or legal guardian of the minor child must read this form and fill it out in full, and the form must be notarized and proof of ID must be shown at sign in. If this form is not completed in full the minor child will not be permitted to ride. This release must be the original copy. Photo copies or black and white copies will not be accepted. All of these guidelines must be followed with no exception.
